New patient forms will be needed. These forms can be filled out online or in our office prior to your appointment. If you have insurance, bring your insurance card and information so we can assist with verifying your benefits.

If needed - We may take X-rays to get a complete picture of your child’s dental development. These images help us identify any underlying issues and plan appropriate care.

Filling out your child's new patient forms in advance will save you time at the office. It's quick, convenient, and sends the information directly to our system, filing it right into your child’s chart. While this is not required, it is a helpful option! If you'd prefer, you can still fill out paper forms when you arrive at the office.
Important: To ensure the forms are correctly tied to your child's chart, please enter their name and date of birth (DOB) accurately on EVERY form. Additionally, a signature is REQUIRED at the bottom of each form.
